New Patient Registration

registrationPlease bring along your NHS medical card to reception. If you do not have a medical card our receptionist will ask you to complete a registration form.  This will help your medical records to be traced from your previous doctor.  (This form may also be used to register as an organ and/or blood donor.) 

Our Practice covers all of the West Bridgford, Gamston, Edwalton, Ladybay, Wilford and Compton Acres area with NG2, NG11 and NG12 postcodes. The practice also accepts patients from the Turneys Quay area in The Meadows.

You may express a preference of Healthcare Professional from whom you receive medical services.  This may be either in general or in relation to a particular medical condition. Any particular preference will be recorded in writing.  We shall try to comply with such a request but need not do so if we have reasonable grounds for refusing to provide services to a patient or the Healthcare Professional does not routinely provide the service in question. 

New Patients 

We welcome all patients living within our practice area and operate a non-discrimination policy. 

All newly registered patients are offered an appointment with our Practice Nurse for a routine health and blood pressure check.  During this appointment you will be asked for information relating to your health and any relevant family history will be noted.
 
Please bring along a specimen of urine in a small scorched out bottle. If you are taking any medication you will need to see the doctor before a repeat script can be issued.  Please bring along all your medicines to this appointment. 

Temporary Residents 

The doctors may agree to see patients who are temporarily away from home ie, more than 24 hours and less than 3 months, and unable to see their usual doctor provided the person is temporarily resident at an address within the practice area.  Please check with the receptionist for the current criteria regarding registering as a temporary patient.
